Domestic Partnership Registry

Cook County's Domestic Partnership Registry enables same-sex couples living together in Chicago and suburban Cook County to formally record their relationships.

The Cook County Clerk's office administers and maintains the registry and issues Domestic Partnership Certificates.

Obtaining a Domestic Partnership Certificate from Cook County does not automatically qualify you or your partner for employer-sponsored benefits, nor does it confer survivorship or ownership rights. Consult with an attorney regarding these and other financial and medical matters.

Records

The Clerk's office will keep an official record of your domestic partnership and any Affidavits of Amendment and Affidavits of Termination that you file. The names of domestic partners who register and the date they registered their partnership with the county are available to the public.
